The Truth About Aesthetic Medicine That Few Doctors Say

Why does Dr. Mandjukova say 'no' to patients?

Dr. Mandjukova, how did your professional path begin and how did medicine lead you to the aesthetic field?

My earliest memories are of treating my dolls - measuring their temperature and giving them aspirin to get better. My professional path began in classical medicine, where I built my foundation as a doctor - with respect for science, responsibility, and humanity. I specialized for 6 years at VMA, after which I started working in an international company in aesthetic medicine. Even in the early years, I realized that medicine is not just about treatment, but also about restoring self-esteem and quality of life. Gradually, I began to take an interest in aesthetic medicine as a natural continuation of patient care. For me, it is not vanity, but a form of prevention and support for inner balance. Thus, my mission was formed - to combine medical expertise with a sense of harmony and natural beauty.

When did the idea of creating your own aesthetic salon first appear? Was there a specific moment that triggered this decision?

The idea appeared gradually, but there was also a clear moment of realization - when I understood that I wanted to work according to my own standards and philosophy. In my practice, I saw that there was a lack of a holistic approach to the patient that combines medicine, aesthetics, and trust. This prompted me to create a space where quality and individual approach are paramount. It was not an impulsive decision, but a result of accumulated experience and inner conviction. It was more of a natural next step than a sharp turn.

How was the concept for Belle Femme born and what was your initial vision?

Belle Femme was born as an idea for a place where beauty is viewed as balance, not as correction. From the very beginning, my vision was to create a medical center, not just an aesthetic studio. I wanted to offer services based on knowledge, experience, and individual analysis, not on mass trends. It was important for me that patients feel understood, not just served. This philosophy has remained unchanged to this day - a natural result, not an artificial transformation.

How bold a move was it to create your own business and what were the first steps?

Starting your own business is always a bold move, especially when you set high standards from the very beginning. The first steps were related to a very clear vision - what I wanted to offer and how to stand out. I invested not only in equipment but also in training and development. At the beginning, the most important thing was to build trust - that is the currency in our industry. I focused on quality, even when it meant slower growth.

What were the biggest challenges at the beginning and how did you overcome them?

The biggest challenge was to impose high medical standards in an environment where the market often seeks quick and cheap solutions. There was also a need to build trust from scratch. I overcame these difficulties with persistence and clear positioning - without compromising on quality. I believed that long-term trust is more valuable than short-term success. This turned out to be the right strategy.

Did you encounter skepticism or misunderstanding and how did you react?

Not really. Bulgarian ladies have a sense for beauty, aesthetics, and modernity and quickly trusted me.

Have there been moments of hesitation when you questioned whether you were on the right path?

Of course, such moments exist for anyone building something from scratch. But for me, they were never related to doubt in the direction, but rather in searching for a better way. In difficult periods, I returned to my core mission - to help people feel good in their skin. This has always given me clarity and resilience. Doubts are part of growth, not weakness.

What lessons did the most difficult periods teach you?

The most valuable lesson is that sustainable success is built slowly and consciously. I learned not to compromise my principles, even when it seems like the harder path. Difficulties also taught me patience, both towards the process and towards people. I realized that trust is earned through persistence, not promises. And that the strongest asset is reputation.

How have clients changed over the years in terms of attitudes and expectations?

Clients have become much more informed and demanding, which is a positive change. Today they seek naturalness in their appearance. They want to look good but without losing their individuality. This requires an even higher level of professionalism from us. In a sense, the market has matured. There are almost no ladies who want huge and disproportionate lips. The Bulgarian woman is a beautiful woman with a good sense of elegance and finesse.

Which procedures are currently the most sought after and what do you think is the reason for this?

Currently, the most sought-after procedures are those with a natural effect and minimal recovery. Patients want to look fresh, not changed. This is due to the shift in aesthetic standards - from 'visible correction' to 'invisible care'. This defines the trends in the industry.

How do you select the new technologies and methods that you introduce into your practice?

My approach is extremely selective and evidence-based. I am not guided by fashion, but by effectiveness and safety. Every new technology goes through careful analysis and training. It is important to me that it brings real value to the patient. This is a responsibility that does not allow for compromises. Every year I attend the most elite medical congresses - from Monte Carlo to Milan to Dubai, to learn the latest techniques and products in aesthetic medicine.

Which innovation in aesthetic medicine do you think will most strongly change the future of the industry?

The future lies in personalized medicine and combined therapies. More and more, we will work with individual protocols based on the patient's biology. Technologies will become more precise and less invasive. But the most important thing will remain the knowledge of how to use them correctly. Innovation alone is not enough - the expert behind it is important.

How do you balance between modern trends and professional ethics?

Balance comes from clear principles. Not every trend is suitable for every patient. My role is to be a doctor, not a performer of desires. Sometimes this means saying 'no'. Ethics always stands above fashion.

What kind of people do you most often meet in your practice and what do they actually seek behind the desire for external change?

I most often meet people who want to restore their sense of confidence. External change is often a reflection of an internal need for balance, for a new stage, for self-esteem. It is rarely just about appearance. This makes our work so responsible. We work with emotions, not just with appearance.

Do you ever refuse a procedure and in what cases?

Yes, and this is an important part of professional responsibility. I refuse when expectations are unrealistic or when the procedure is not in the patient's interest. Also, when there are medical contraindications. Trust is built through refusal as well. Sometimes the best care is not to perform an intervention.

Can you share a real story of a client that impressed or inspired you?

I have had many cases where a small change leads to a great internal release. One of them was a patient who avoided social contacts for a long time due to insecurity about her appearance. After a carefully selected approach, she not only changed her appearance but also the way she perceives herself. The most impressive thing was not the result, but the confidence she built. These are the moments that give meaning to the profession.

Has there been a case where a small aesthetic change led to a significant personal transformation?

Yes, and this happens more often than people assume. Sometimes a delicate correction unlocks a change in self-esteem and behavior. This reflects on the career, personal relationships, and the overall presence of the person. However, it is important that this change is natural and harmonious. That is where the role of the doctor comes in.

What are your future plans for the development of Belle Femme?

My plans are related to development, but without compromising on quality. I want to expand the training activities and pass on my experience to the next generation of doctors. Also, to implement even more precise and personalized methods. The main goal remains the same - sustainable development and trust. Growth must be meaningful.

How do you envision the development of aesthetic medicine in Bulgaria over the next few years?

I expect the industry to become more regulated and professional. Patients will demand higher quality and safety. This will advance the true specialists. There will also be a stronger focus on natural appearance. This is a healthy direction for development.

If you could go back to the beginning, what advice would you give yourself?

I would tell myself to have even more patience and trust in the process. Not to rush, because sustainable results come with time. And to continue following my principles, even when it is difficult. Ultimately, it is these principles that build identity. This is the most valuable capital.
